    Drivers are responsible for ensuring wheelchairs are properly secured. Failure to secure can lead to wheelchair movement and serious, preventable injuries to (wheelchair) passengers.


    Abrupt transit vehicle braking can throw an unrestrained wheelchair passengers forward, placing them at increased risk of harm. Proper wheelchair securement and occupant restraint systems will protect wheelchair-seated passengers during normal vehicle driving and vehicle crashes.


    Safe loading and unloading wheelchair passengers is part of the driver’s duty, missteps and shortcuts can result in serious falls and injuries to wheelchair passengers.


    Wheelchair passengers are typically more vulnerable due to their visible or invisible disabilities. Drivers transporting passengers in common carriers owe these vulnerable passengers a heightened duty of care and understand how to use the safety systems on board a vehicle to safely transport those passengers seated in wheelchairs.

    Medical devices are used by a wide range of patients, caregivers or health care professionals. When a medical device is not following a user-centered design process or evaluated upon safety and effectiveness, unacceptable residual risk of harm to users may still exist and cause preventable harm to users of the medical device. 


    Improper use of medical equipment can result in complications, infection, or serious injury.


    Poor positioning in medical devices can lead to instability, falls, or long-term harm.


    Improperly fitted positioning devices can contribute to pressure injuries, many of which are preventable.


    Those responsible for selecting, setting up, and introducing medical devices have a duty to promote safety, usability, comfort, and proper fit for users. Users, however, are responsible for understanding how to maintain and adjust their devices according to manufacturer instructions to ensure ongoing safe and effective use.
